Small lump with crack in anus after anal sex.

Resolved Question:

I am a 22 year old male, with a clean medical history, except what may be slightly relevant here is that I have previously had fissures, hemhorroids and migraines that at times occur when I need to deficate. I recently had two blood tests for immigration purposes; both clean.

Six days ago I engaged in receptive anal sex with a parter who had only one previous partner a few years old patientago. I have done very light touching and kissing with others but no other sex recently. I had sex with him a few times before this and had no resulting issue. I also had no unusual pain afterward.

1.5-2 days after our last meeting (4.5 days ago) I awoke with my inguinal lymph nodes so swollen you could see them, and sore to the touch. I also felt like I had a sort of swelling in my rectum/colon, sort of like a hemhorroid. White mucus discharge accompanied my stool, and a discharge in general during the day.

I waited 24 hours to see if my body would clear it up before I took one Cefixime 400MG and then Doxycycline 100MG twice daily - I am about to take my eigth dose. It has had little effect, besides perhaps reducing swelling of the lymph nodes, which do not hurt as much and no longer hurt me when I walk. I also had a feeling of soreness in my left upper thigh/right cheek that kept getting worse for two days but has begun to subside. I have fairly intermittent sort of ache behind my testes, which intensifies when I strain to pass a stool.

Within a day or two of the start of this I began to have worse and worse hemhorroids and pain in my rectum, so that it would very much hurt to cough or sit or flex that muscle in any way (such as during urination) and makes the passage of stool incredibly painful. On the second day, I applied a more natural aloe vera / peppermint ointment and by the night had a rash with large bumps on the inside of my crack. I stopped using it and switched to using Cortisone-10 to relieve the rash and a more standard hemhorroid cream with suppository.

The rash still got worse but stayed in that general area, while the hemhorroids for a time felt to improve, they get worse than ever before when I need to pass a stool - and there has been no lessening of the mucus and my nodes are still swollen.

How should I change my treatment strategy? What could the STD be that is accompanying the proctitis - or could this be just a general proctitis infection?

I fear going to a hospital here and having a doctor report me for homosexuality.

Possibly inconsequential:

I may have had brief chills the second night, or some slight, brief sensations/changes in feeling in my arm/leg, but they were limited and I do not include them in the primary symptoms.

I understand your concern.
With a previous history of hemorrhoids, fissures, it is very easy to pick up an infection from any kind of anal penetration as it makes the anal mucosa prone to injury.
You could have picked up a prostate infection, as the symptoms of tightness in the thighs, and pain in the testes are all indicative of the same. Though you took the right treatment, it does not seem like Doxycycline is working.
You will need a separate antibiotic like Tab. Cotrimoxazole ( TMP-SMX ) twice daily for a period of 2-3 weeks, which works well for such infections.
Regarding the bumps and the rash, you need to get tested for herpes. as herpes presents in this manner. You need to get HSV-2 IGM test done, and you can revert back to me.
Please do not worry about your sexual orientation, no one is going to question you on that. Kindly get the tests done and get back to me.
