Breathlessness when physically not active. HEART PROBLEM?

Hi, I am Murali, a 39 year old man from Mumbai. I am facing breathlessness since about 1 year. This happens when I am not active or am alone or am thinking about some worrying incidents. However, when I am physically active,like doing skipping or playing badminton, there is no problem at all. I can push myself very hard physically, like I can do 200 skips at a stretch, run 10 km marathons without a problem. However, when I am relaxed/ sitting/ thinking, I feel breathlessness and heaviness in my heart. There is no problem during sleeping. I don't have any bad habits like smoking or drinking alcohol, and I don't have any medical history. Please let me know is this breathlessness and heavy heart a physical problem? if yes, heart or lung? or is it anxiety related problem? By nature, I am an introvert and worry a lot about things. Thanks.

From your history, it is implied that you have anxiety related issues and that can cause symptoms like breathlessness, palpitations, e.t.c
I would still advise you to get an ECG done, and provided it is normal, the diagnosis can be established as anxiety disorder and we can proceed further with management.

Thanks Dr. Satish. I have a question: Does ECG indicate any underlying physical problem? like for example arteries blockage or cholestrol or tumour or cancers, etc.? So far I have just checked my blood sugar, the random sample has the value 93. My height is 5'11", weight 60 kg. Is it posssible that cholestrol or arteries blockage may lead to heavy heartedness when sitting steady and the problem will be relieved when doing a heavy exercise? Thanks.

ECG will give a good idea of the status of the heart and heart valves.
If ECG is normal, at your age, we will understand that the heart function is completely normal.
And no, with your height and weight I do not think there is any risk of atherosclerosis.
