Suggestion for HIV test

please suggest as today is the 12th day post exposure if go for HIV 1 RNA QUANTITATIVE, REAL TIME PCR WITH CD3, CD4 / CD8 (IMMUNE DEFICIENCY PANEL 2) MARKERS will it be conclusive

Hello and thankyou for approaching DoctorSpring with you query.

It is too early to test for HIV as your exposure has been recent.

I would advise you to do HIV Combo (Antigen/Antibody) test for HIV at 3-4 weeks from the time of exposure.

Or The Antibody Test for HIV at 6 weeks from the time of exposure.

(The HIV 1 RNA Quantitative, Real Time PCR is done in a patient who is already diagnosed as HIV+ , to asses the prognosis of the disease, monitor progression of the HIV infection or to monitor the effect of the antiretroviral drug therapy for someone who is already on treatment for HIV. )

So the earliest you can go for is HIV COMBO TEST (Ag/Ab)- also know as 4th generation. I recommend this.

*DO NOT go for the RNA tests as it can be false postive.

There are no symptoms but its the anxity and trauma which kills me can the test detects the infection on 13 th day if exists and can have negative report is good indicator please reply soon

What kind of sexual exposure you had ?
Unprotected sexual encounter ?

At Least wait for 1 more week and go for the Combo test. That will be you best choice.

Patient replied :

I have a unprotected sex exposure with a CSW however I rarely I had any exposure and this is the first time I had a unprotected exposure and as per the findings the the transmission from female to male is having a probability of .1% if the female is infected and I am not aware of the HIV status and the ejaculation is also not inside the vagina and as you said test to be taken after one week if neagtive/ non reactive is good indicator if taken at the 23rd day please reply

In that you do need HIV test.

A test taken on 23 day (if it is a combo test) is a good indicator of your HIV status. If it is negative you can be reasonably sure that your are indeed negative. However you again will need to do a confirmatory test after 6 weeks and then 18 weeks. (not combo test, the common ELISA test will do). Thank you

One more thing doctor if the person is having any symptoms and goes for the test may be on 14th or 15th then does the infection is detectable and you havent answered that is it right that probability of transmission of infection from a female to male is very low as low as .1% are the findings right hope to be hearing from you soon

On 14 th or 15th you can for the test, but in that case the result will not conclusive. You will need to repeat it on 4th week again. If there is antigen or antibody already it will show in test. So you can choose to do it, but has to be repeated if negative.

The risk is lower in males. Risk is somewhat like 5 per 10,000 intercourses. (10 cases if it is female). But since youe exposure was with a CSW, the risk is higher.
