Treatment for CHRONIC COUGH with DIZZINESS.

I have chronic cough for last four years. What could be the reason? I have no asthma, tb, allergies or any other lungs related ailments. The thing ENT specialist could find was that I had synys and I had operation for that three months ago. We were hoping the cough would go but I'm still coughing.

It all started when I had bad flu 4 years old patientago. I got better otherwise but the cough wouldn't go. I fee kind of scrach kind of sensation and this trigers the cough. The cough is so bad that I feel dizzy when I cough. I don't smoke and I don't drink regularly. I am healthy otherwise, no weight loss, no loss in appitite or any physical strength. Only lately I'm starting to feel depressed because I stand out in any social events because of this cough and you can guess my trouble at work place.

I can understand your problem.
You are suffering from chronic cough for 4 years old patientIt can be due to various reasons. Common are allergic pharyngitis, acid reflux, chest infection etc I want more information to get correct diagnosis including- Dry/ productive. Colour of sputum? More in day or night? Associated symptoms in including breathing difficulty/ chest pain/ any voice change? Whole year or in a particular season?
Do you have any wheezing sound?
How are your nasal symptoms now - sneezing/ nasal obstruction/ nasal discharge/ headache?
Do you have chest xray, sputum test, blood test including ESR.
With available information most likely diagnosis will be allergic pharyngitis.
You can start tab. Monteleukast 10 mg+ Levocetrizine 5 mg in night for 10 days.

Patient replied :

As per your advise I went to my GP and get him to prescribe Montelukast (as sidium) 10mg tablets which I'm taking for next 28 days and azithromycin (which I was told is a cheaper and equally benificial substitute for Levocetrizine). I'm to take azithromycin for next three days.
My cough is not dry and it is worse when there is change in temperature (both way) even worse when there is sudden drop or rise in temperature. It does not matter which part of the year. Also, I faintly feel there is some abstruction or information at the left part inside my throat. The information or the swollenness is felt more on my bad days when I'm coughing constantly.
Multiple blood tests revealed no infection, no allergies (did blood test and skin test), no chest related infections, no astema (done all different test), no wheezing sound, no breathing problem, no chest pain, no voice change.
All my syntoms related to synus are gone but when I'm coughing I still get teary eye and running nose.

Thank you for the follow up.

With all information, most likely your diagnosis is allergic pharyngitis .

So continue monteleukast 10 mg

Azithromycin in antibiotic and it is totally different from levocetrizine which is anti allergic medicine ( antihistamine)

So I will advise you to start levocertrizine 5 mg for two weeks.
