Back, abdominal pain and kidney stone. CT.

Hi,
for two months I have back pain all over, followed by right abdominal pain, I went to the chiropractor for a month, no improvement.

I have a kidney stones 6 months ago , after peeing the stones , everything was good and it came back two months later.

I have just done a CT scan with contrast and blood tests.

I still have back pain come and go.

I have IBS.

I understand your concern.
There is no relation between the presence of kidney stones and the type of abdominal and back pain you have been suffering from
As per the reports , you are giving urinary tract infection with turbid urine and presence of bacteria and rest of all the reports are normal
You need to get an urine culture test done.
Do you have diabetes , hypertension , previous passage of stones or urine infections etc ???

Hi Doctor,
That was the first time I had a kidney stone, I was working out a lot and drinking protein shakes and food that contain a lot of protein, and taking creatine, I didn't drink a lot of water. but I stopped once I got the stone. I don't have any family history of kideny stones.
I don't have diabetes, although everytime I went to the doctor and took my vitals I had a high blood pressure.
attached is my previous blood test.

Thank you for the follow up.
I went through your previous reports and comparing it with your present reports the only striking factor is the UTI which is still there and hence i advised Urine culture test.
The kidney stones as now is evident was due to excessive protein intake, without adequate water intake. Now the lower back pain and the abdominal pain can be attributed to the uncorrected UTI.
Did you receive any antibiotic/ or any other treatment for your UTI last time?
Do you have any burning micturition or any blood in the urine?
The kidney in the recent report is fine now, and there is nothing to worry about the kidney.
However once the culture tests are available, treatment for UTI has to be initiated right away.

Hi Doctor,
Back then when I did the first lab test, I had the kidney stone in me, and they didn't know if it's a stone or not, so my doctor gave me nitrofurantoin for 5 days.
This pain started two months after I passed the stone.

Hello.
Thank you for the follow up.
Nitrofurantoin was given to you as an antibiotic treatment for your UTI. It has no connection with the kidney stones.
Your urinary tact is still infected and that is causing the symptoms. Kindly get an urine culture done again which will show the specific organism.
Once you give your urine for culture, you can start on antibiotics. I will advise
1) Tab. Augmentin ( Amoxycillin + Clavulanic acid ) 650 mg twice daily for 7 days.
If you start the treatment before giving the urine for culture, it won't show what organism is present and hence please get a culture before the treatment. Since last time multiple organisms were present, it is important o know which antibiotics will work on the organism. I am giving you augmentin till the reports come out, as it acts against most organisms and can be used as prophylaxis.

Hello.

The report you have attached seems to be an old report, which you had sent earlier and there is no urine culture test reported in that. Most of the tests are normal. You don't need to worry. GFR is normal. Electrolytes seem to be on the higher side, so you need to be well hydrated. Drink at least two liters of water everyday, if not more.

Your blood sugar level is also on the higher side as well as ketones are present in your urine. You may need to get a HbA1C test done to rule out overt diabetes,

Few bacteria are seen in urine and you should get it confirmed by a Urine Culture test as presence of bacteria in urine is abnormal in men your age.
The kidney stone you had must have caused infection in your urinary tract. Despite the absence of the stone now, you can still have infection which should be treated by antibiotics as I mentioned before.

Take care of IBS also simultaneously and talk to your doctor locally for a urine culture. Please get back to me with your culture report and I can help you further.
