Feeling tired all day, mood swings on ZOLFRESH. Treatment?

Hello Doc,

I am 30 yr/F, Single, working as an IT Consultant in Noida. I was diagnosed with Hypertension in April 2013 for which I am taking some medication mentioned below
1. Olmark 40 (H) - One tablet a day
2. Lipicure 10 - One Tablet a day
3. Zolfresh 5 mg - SOS

I have been taking the above medicines regularly as per my latest prescribed medicines for over 4 months now.

Now, I would like to explain my problem. For almost a year, I have been having constant mood swing issues which have increased a lot off lately. It began with over sleeping and getting irritated with family members on small things, always feeling tired, no mood of dressing up and getting out of bed on weekends. Initially, it happened rarely and it appeared mostly during winter season last year. I became well during summers and did not have any mood swings/depression.

Since last 2-3 months, this seems to be happening again. My mood swings happen almost everyday now. I feel tired most times of the day and do not feel like going out of my house except for going to work which is necessary. I do not feel like chatting with my friends or talking to family members. I do not initiate conversation with team members at work unless required. While, I try my level best to behave normal and happy and also make efforts to control my anger and depression, sometimes I burst out in anger. My anger outburst happens mostly with my mother. There have been more than 10 times last month that I fought with her over something I realized was my mistake and had to apologize to her later on.

I feel guilty and sorry after behaving like this with anyone however I cannot seem to control my anger. I sometimes feel I am responsible for making my family unhappy and I should leave the house. But the next moment I control my thoughts and get back to normal and try to make up for my ill behavior.

I have no difficulties doing my work in office, cooking or other household work however I do not feel like doing anything. I do not feel motivated to do anything and if given an option, I would prefer sleeping all day, do not take a bath and do not talk to anyone. I try my level best to motivate myself and concentrate on work and other things like shopping, meeting friends.

Please suggest if I should visit a Psychiatrist or counselor for help.

I have gone through your question in detail and I can understand what you are going though.
From what you have stated it is quite clear that you are having a depressive episode. Depression can occur with regular stress and it can also occur biologically without any stress as well. Winters are generally more depression-prone periods. You need proper treatment for the same. There are various effective therapies for depression like fluoxetine, venlafaxine, bupropion, mirtazapine or escitalopram and they are all very effective and mostly safe. You may also require some benzodiazepines like clonazepam or lorazepam for a short period of 2-3 weeks to reduce the anxiety and help with your sleep.
If you are averse to the drug therapies then there is also an option of cognitive behavioural therapy. This therapy is taken by either a psychologist or a psychiatrist in which the therapist identifies impaired cognitions (Thoughts) and try to correct them. They are 15-20 min weekly session and there should be around 8-10 sessions to help you with your illness.
You may visit a psychiatrist initially and if the symptoms do not settle down then you may consider augmenting your treatment with the counselling as well.
