I have been sleeping with my partner for 4 months. For the first 2 months everything was fine, however in the last two months we have had problems. Each time I have sex, the skin on my foreskin has 5 to 10 small cuts. They are sometimes very painful and I can't pull my foreskin back. I do not generally have a tight foreskin and we have tried lubricants.

Two most common cause for such cuts in the foreskin and glans are (1) Inadequate lubrication and a (2) Fungal (Candida) infection. Since you had everything normal in the initial months and since you have tried lubricant without any benefit I suspect a Candida infection.
I would like to know where there is any redness over the glans penis ?
Also is there is any history of high risk sexual behaviour ?
Any history of Diabetes or immune suppressing condition ?
