hi. my friend just send me a picture of what to be like a lesion or red sore on his penis head. could it be balanitis or some kind of STD. He said that the sore appeared around 2-3 months ago. He said the red spot on his penis head is not itchy but getting irritated when wearing tight underwear during hot day.

It seems to be genital herpes. It is caused by the HSV virus and is sexually transmitted. I would recommend taking anti-viral medication like valacyclovir.
Application of mupirocin cream twice a day would be helpful.
