Past Crohn's and now gas pains and fainting spell

Resolved question:
I have had crohns disease all my life. Half my large bowel was taken out when I was 24yrs. Iam now 48. For many months I have gas pains, foul smell when passing gas, lack of concentration, dizziness, no appetite for days then I binge eat, diarrhea everyday, blurry eye sight, last night I fainted thought I had a seizure could have just been twitches as I fainted, before fainting I was extremely dizzy broke out in a sweat was dripping water off my body, iam allways tired. I do not have a doctor of my own they are hard to get here. When I go to emergency they just send me back home to rest

I would like some additional information so that I can provide my best and learned opinion.

1. Are you any medications for Corhn's ?
2. May I know what kind of congenital heart disease you are having ? Is it corrected / treated ?
3. Have recently checked your basic blood panel ? Especially the Blood sugar levels ?
4. Is there any weight loss ?
5. What happened following the fainting episode ? After how much time did you recover ? Was it a spontaneous recovery ? Were you alright after the recovery ? (any weakness ? any blurred vision ? etc)

Patient replied :

No medication for crohns. Not sure on the heart disease but did have an ultra sound on it and was told that I have a murmur and my vents she called them gills like the fish gills are overly large abnormaly large and she said there is plaque in my arteries. This test was done about 12 years ago no one has checked since. Three month ago my step father whom is diabetic checked my sugar levels and everything was normal. As for fainting I became extremly dizzy I sat down broke out in a drenching sweat , felt like passing out in sitting position, felt like pukeing couldnt concentrate, trouble moving my limbs arms legs head felt as if they were numb everytime I lifted my head I was extermly dizzy, felt hot as if I was in an oven, eyes were very burring hard to see felt as though I was looking down a tunnel. I also passed extremly sewer smelling gasses. I thought I had to puke so I pushed up from my chair procedded to walk to washroom I bounched off the fridge, cupboards going back and force bounceing off everything my legs kept going in different directions, extremely dizzy couldnt focus my eye sight it actually felt like someomeone pushing me back and force off balance, then I fell backwardsbody twitching fell down hard back of my head banged off the door and then the floor I also bite the inside of my mouth very hard, not sure how long I laid there not sure when I came to.as all this was happening I thought I had to have a bowel movement really bad I was feeling very weak and drained. Someone gave me an ice pack I put it on the back of my neck after abit my body temp came down and the sweat stopped dripping off me. Sorry it took so long to explain. I have had a few of these spells before I went to emerg those times. They docs gave me shots of morphine, then would send me home the same day to rest. Not once have the done any testing on me in the last 5 years, which I find stupid because a doctor about 9 or10 years ago had done a scan on me and found what he believed to be a non cancerous lump on my brain. So I think those reports should be in medical file but these doctors are not looking at it at all and just sending me home
to suffer the headachs that I also get with these spells. Im sorry for venting but im very frustrated. Thanks so very much for your time.

I have discussed with our team Neurologist Dr.Sudhir Kumar
Here is what we think :

1. The episode could be a seizure. If this recurs you will need an evaluation for sure. Disucsss this options with your Doctors. I would recommend a CT brain, an ECHO (as part of your cardiac evaluation ) and a EEG. If CT is inconclusive you will have to go for a MRI. I know this is a lot to ask but there is not other way unfortunately.

2. The foul smell could be bacterial over growth (SIBO). This can be treated with an antibiotic like Rifaximin. Once the antibiotic course is over you can take a probiotic capsule for 3 days. I believe this foul smell issue can be easily treated.

3. You should take a Vitamin supplement once a day. B complex vitamins

If you have medical access I would strongly recommend evaluation for the seizure. Cardiac evaluation should be part of it. Hope this helps. Do feel free to write to me / ask followups / clarifiations
