Now i am just curious here, A Tension headache can cause tenderness across the scalp/top of the forehead. Now does that tenderness involve like a slight scrape feeling running over the surface of the skin?

Tension headache can definitely cause tenderness, across the scalp/ top of forehead, or even on the sides.
They are very similar to migraine and if it persists, it can be associated with nausea or vomiting.
Where exactly are you refering to when you mentioned slight scrape feeling? Is it the scalp? That's unique, however anxiety does have a lot of various ways of presentation.
