Treatment for pressure in scrotum without discharge from penis

Resolved question:
I am a man and received oral sex from a man. about 5 days later I had pressure in my scrotum and felt like I had a discharge from my penis but did not. I tested negative for chlamydia and Gonorhea. I was treated with doxy 100mg twice a day for 7day and symptoms did not go away. Then had flagyl 2000mg x one dose. Seemed to help but 2 days later it is worse. Still no discharge. I am wonder what the next treatment would be.

I understand your concern.

Your symptoms include a pressure in the scrotum and a sensation of a discharge from the penis. Since tests for both Chlamydia and Gonorrhea were negative, you require further evaluation.

I would require additional information in order to provide my best opinion.

1. Did you get a urine routine and microscopy test done? If yes, what were the results?

2. Do you have any fever? Any rash / skin lesions on the genitalia?

3. Do you have any swelling or lumps in the groin?

4. Do you have any burning while passing urine?

Patient replied :

The answer to all your questions is no. The urine was negative. I do have a small gray like area on one of my testicles. The pressure seems to be relieved right before I urinate. No increase in frequency.

Thanks for writing to us again and giving us additional history.
Pressure in the scrotum/ scrotal pain, especially in young adults is caused due to STDs, most commonly Gonorrhoea, and Chlamydia. Since you have tested negative for both, then there is nothing to worry about. Other causes of such scrotal pain doesn't seem to be the cause in your case, such as orchitis or testicular torsion. You have also received treatment covering both the infections. Doxycycline is the first choice medication for most STDs. So we can safely say it is not due to STD.
Another thing that could have caused your symptoms is UTI. UTIs can be asymptomatic at times, just causing slight burning while urination. So if possible i will advise you to get an Urine culture done, which will indicate any growth in your urine and will also show which antibiotic is most suited for the infection. Apart from that, there is nothing alarming as of now. Since you have already received two courses of strong antibiotics i won't put you on any other antibiotic till your culture report comes. If your symptom does not subside in another couple of days, then i will advise you to get an Ultrasound of the Scrotum done. That will give us a clearer picture.
Also if there is any open wound in your scrotum, apply T. Bact ointment. It will help in recovery faster.
