I need advise on my below semen analysis report and doctor prescribed to use Evion LC tabs for 30 days (daily twice morning and night).
Please explain me in detail what needs to be improve in report.

In your case, the semen count is normal, and shape of sperms is normal, but the sperm motility is below optimum.
This can be due to various reasons - stress, abstinence, ageing, chromosomal defects, hormonal deficiency, blockage of ducts carrying the sperms, infections, toxins, radiation, excessive local heat, radiation, chemicals etc.
You should repeat the test after 30 days of taking the antioxidant tablets prescribed to you.
If repeat testing shows the same problems, you need a more detailed evaluation -
You also need a complete internal exam by an Andrologist.
You also need scrotal ultrasound.
Also, you need to have a semen culture as semen sample is showing pus cells.
Smoking and drinking should be strictly avoided.
