Hi. I have always suffered from anal fissure and used apple cider vinegar and nitroglycerin paste. But it's not helping this time. Please help. I have attached a photo of the condition. Should I start antibiotic.

I saw the picture and it appears like internal haemorrhoids.
Fissure is a crack like condition inside the Anal canal. It causes pain while passing stool and bleeding sometimes. As it is inside, it cannot be seen clearly.
In haemorrhoids, there will be more bleeding.
Please consult a surgeon in person to have it checked. Please confirm whether it is a Fissure in ano or haemorrhoids or combination of both.
Your stool should be soft. For this you should take high fiber diet, drink plenty of liquids, eat fresh fruits and vegetables.
There is no role of antibiotic since there is no infection involved.
You should take stool softener solution like Lactulose or Sodium picosulphate in the night at bed time.
If there is no response to above measures, Surgery has to be done.
