Regarding your case, I don't think the doctor would be able to take out the granulomas with liposuction only. This might sound depressing but this is true. Granulomas are well fixed to the body and are quite resilient to liposuction techniques. Therefore, the only way of managing these is to remove them by open excision. Contouring with fat can always be done but excision is the mainstay in the removal of these granulomas.
Silicone granulomas have a low propensity of causing serious health problems but if you are concerned with the appearance of these granulomas it is preferable you ask a surgeon to remove them by the open technique making small nicks in the regions of the granulomas.
