Difficulty concentrating and memory and speech problems

Resolved question:
I have had the following symptoms and it is becoming a major problem now:

As a child
- had a hard time concentrating
- difficulty reading and speaking
- anxiety
- very active child
- difficulty socializing excluding close friends
- very poor memory
- A lot of daydreaming

Teenager
- hard time concentrating
- slept a lot more and felt depressed
- very poor memory
- anxiety
- hard time speaking and reading. When I tried to read my concentration span was very short
- a lot of daydreaming
- hard time thinking of the word im trying to say

Adult
- hard time concentrating
- feeling of depression when I am very tired
- Energy level is very low mentally and when I am really tired I feel depressed
- daydreaming
- in big group settings when i am required to talk, my anxiety gets really bad where i feel like i cannot talk
- brain fog
- difficulty talking some days (very bad some days)
- difficulty reading, trying to read puts me to sleep
- very poor memory
- hard time thinking of the word im trying to say. Better in the morning but as the day progresses it becomes a lot worse
- difficulty reading menu's
- odd week (very rare) I have a good weak of energy and concentration, physically more active
- hard time understanding when I am speaking to someone one on one
- forgetful
- when im writing sentences i make a lot of errors and one sentence can take me a few minutes even though very simple.
- sometimes i feel dizzy all of sudden - energy just fades

Tried caffeine pills for the first time this year, the pills worked wonders (memory good, energy, good concentration) at first but after 2 weeks went back to how I was. Caffeine pills have not worked the same since.

Went to a walk in clinic and the doctor prescribed Cipralex. Best I felt in past 10 years (memory good, energy, good concentration) but after 3 weeks the affects faded. Doctor tried to increase dosage but went back to feeling tired all the time and symptoms stated above all came back.

I have done blood work for thyroid, vitamins, diabetes, hemogloblin. All were ok. Iron was alittle low but other than that everything was fine.

I have gone through your question in detail and I can understand what you are going through.
Firstly I would like to appreciate the way you have asked your question. The whole picture is so elaborate in front of your eyes.
Your symptoms suggest that you could be having an Attention Deficit Hyperactivity Disorder, predominantly inattentive type with co-morbid redcurrant depressive disorder. Caffeine is a stimulant and thats why it has helped you.
The other possibility is social anxiety disorder. But it seems to be less likely.
The drugs that should suit you best are methylphenidate as a stimulant and Bupropion as an antidepressant. These drugs in combination will be activating you as well as take care of depression without issues of drowsiness.
ADHD was thought to be only childhood disorder but now its understood that the symptoms do persists in adulthood as well. There is a remarkable benefit with methylphenidate.
Being an online forum, I wont be able to prescribe you these medications, but you can discuss this with your psychiatrist who would be more than happy to help you.
