My mother 74 years old patienta diabetic patient since last 27 years old patientshe take insulin injection 3 times a day and her diabetic is in good control always Glyco in range of 6 and 6.5 we monitor and accordingly its well maintained. She is suffering frequent Urine Infection for which we give repeated antibiotec as per culture report. She taken Revotril 0.5 mg sleeping pill 2 and half pill every day. As of now her current problem is that she has breating problem once she walk a little with walker support she gets fast breathing. her cardiac reports are normal lungs are well in function but 2 main thing breathing problem and she feels lazy and cant do anything she became hopeless about her life she always feel i am dying

I understand your concern.
The good thing is for somebidy who is suffering from diabetes for 27 years, glycated hemoglobin in the range of 6-6.5 is actually very good. But one of the major issues with Diabetes is it completely slows down the patient over time. Especially those on insulin therapy, become slow once they start aging. Regarding her breathing problem, did you get a chest x ray done? You said her lung condition is fine, has the pulmonary function tests been done?
What about her blood pressure? Is it under control?
Any cause of recent depression for your mother? Apart from her general condition?
Also is her ecg normal?
Who recommended Revotril to her? What dose was recommended to her?

X ray done Report: Poor Inspiratory effort is noted and therefore lung fields are poorly expanded. and ECG has some STT Changes L1 av L1 V2 V6, thats since last 2 years old patientsame. Her wieght is bt=etween 109 to 112. Revotril given becasue she cant sleep. She also feel uneasy at night while sleep. She takes DAILY Insulin thrice, Pentodac 40 mg , Niftran, Ditide, Urimax, Dipplatt 75, Nitrocontene, Tryptomer, Reseday 10 mg for high cholestral Gabapin and Revotril 0.5 mg 2 and half tab, (Tayo 60 k for vitam d ) once a week for 6 weeks now and She was taking Gemfost once a week but now she does not toleate so we stop since last week for the osteprosis thre is no medicine given

Poor inspiratory efforts can be due to restrictive lung disease, COPD, asthma, pneumonia, CHF, e.t.c
One more thing that can be causing her symptoms is her body weight.
She will have difficulty in breathing with that kind of weight since also because of her age, her lung function will be slightly compromised.
I will advise the following medicine for her :
1) Tab. Salbutamol 4mg 2 times a day. If she tolerates well, then it can be given 3-4 times a day. You will need a prescription for this.
Also the reason of her constant laziness, can be attributed to her weight and the Ritrovil, since she is taking close to 3 tablets a day; she is expected to be sleepy the entire day. Kindly consult the doctor who gave her the medicine for a dose alteration. Without any physical activity, she is going to put on more weight and that won't be helpful.
