Patient replied :

I had sex with a sex worker. She supplied the condom. The outside wrapper was worn, it was stored in her back pocket. I opened it with my teeth, but made sure not to tear condom. I had protected vaginal intercourse. I took off wrapper, then masturbated after she left my house. However, I did not wash my hands.
My question is, could I have contracted HIV in the situation described? What if I had vaginal fluid on my hands when I removed the condom? I had severe migraines 10 days later, with diarrhea. I was scared of giving HIV to my girlfriend, so we only had protected vaginal sex, and unprotected oral sex. I tested for HIV 27 days after incident and it was negative.

I am very frightened. My girlfriend just got a rash on her clavicle. It looks patchy and red. (She is fair skinned). Her rash is not bumpy or raised. It is itchy.

The incident you have described is a very low risk ( for all practical purpose NO risk) scenario. HIV will not transmit via skin. It needs open wounds or abrasion. There should be fluid-fluid contact for HIV transmission. So unless you had fresh wounds or abrasion which was exposed to the vaginal fluid in your hand you need not worry.

The symptoms you have mentioned (and the rash on your girlfriends clavicle ) is not suggestive of HIV. If you have done the antibody test 27 days is bit early. You can repear the test after 4 weeks from now. This is not really necessay, just for peace of mind if this keeps bothering.

I understand not getting HIV through my hand. But what about masturbating with HIV on my hand? I would say it was a 4 minute delay between removing the condom and masturbating with unwashed hand. Could HIV live on my hand, and be absorbed through the head of my penis?

This would also be low risk. As there is no direct contact (from the partner to your head of penis). Also unless there is abrasion or wound at head of penis the risk is less as menitoned in the previous reply. (During penetrative sex some micro abrations occurs).

So practically your risk is very low.
