There are quite a few conditions which presents with these symptoms that you’ve mentioned to have, such as inguinal hernia, genital herpes infection, and other neurological conditions. So to narrow down the diagnosis and in order to help you out better I will need more information from you. It will be great if you can give me these additional details.

1.       Do you have a history Diabetes Meilltus?

2.       Are there any blisters/sores that are present in the groin area?

3.       Did you have any injury in the abdomen or groin anytime in the past?

4.       Do you have any swelling/lump in the legs or in the groin?

5.       Do you have burning sensation/pain when you urinate?

Patient replied :

The answers are no to the first 4 questions. Sometimes there is a minor burning when I urinate and I seem to be urinating a little more frequently and the urine is a little less yellow than normal. I had a urinalysis and it came back negative. The main discomfort is around my left testicle and lower abdomen and it seems to radiate down my inner thighs at times. I do physical labor at times, but I do not remember hurting myself.

From the description of your symptoms, it most probably looks like inguinal hernia to me, it occurs when there is weakness in a part of your abdominal muscles leading to protrusion of a small part of abdominal content through it. But I’m not very sure since physical examination is extremely mandatory to diagnose this condition. Usually, there will be a small lump or swelling seen in the groin area, but sometimes burning sensation precedes the appearance of the lump as it could be in your case. Try coughing voluntarily few times and keep your fingers in the groin area just above the scrotum if you can notice any bulge in that area then it could possibly be inguinal hernia. However, if you are able to decide, then you should consult with GP who can perform these examinations thoroughly. Urinary tract infection is very less likely, since your urine tests have come negative, but, if you persist to have pain or burning sensation then you will have to repeat the urine culture test. And lastly, kidney stones might also mimic these symptoms sometimes. All of these conditions will require help from GP, so I would suggest you to consult with GP who will be able to assess this condition further with diagnostic tests and physical exam. You need not worry, none of this is a serious problem, and it will be completely treatable.
