The ecg that you have attached shows ST elevation in leads V1 to V3 and is suggestive of an anterior wall myocardial infarction. Such changes can occur in an acute heart attack or in case of an old heart attack if the area of heart attack is seriously damaged . If the patient doesnt have a history of heart attacks in the past it means that the changes might be recent.
If the patient does not have a history of heart attacks in the past I would recommend that you take the complaints of patient of gas in stomach seriously and evaluate him for an ongoing heart attack given the ecg changes.
If the patient already has a history of anterior wall myocardial infarction then you may choose to ignore the ecg changes. Such changes can also be seen occasionally in young adults and we call it as early repolarization pattern but that seems less likely.
So it would be better if you could get medical help and show him to a cardiologist or a physicians so that we may rule out acute myocardial infarction in his case. A trop I or Trop T test can be done to rule out a heart attack.
