The changes you are experiencing around the opening are the changes of BXO - Balanitis xerotica obliterans. As of now nothing needs to be done as it is in the initial stages and may regress by itself.What exactly causes Balanitis xerotica obliterans is uncertain. However, if it progresses then local application of steroid cream may be to be considered ( Clinical efficacy has not been proven).
