Why am I feeling suddenly tired every month with normal blood tests?

Every month, for no apparent reasons, I am feeling suddenly tired. It is like a step function. I have no other symptoms: no sore throat, no digestion issues, nothing. I do not have a specific stressful even that triggers it. However, it happens approximately on a monthly basis. Each time it happens, it last for 7-8 days. I had my annual exam and my doctor checked my blood. Everything was normal: no anemia, thyroid was OK, cell counts, etc. Everything was within the range. I eat well and exercise 5 days a week for 30 min. What could I possibly have? What are the next exams I should do?

Thank you for the reply. Here are my answers 1 - I am male (38) 2 - It does not happen at the same time. The last 4 times started on 4/7, 4/27, 5/17 and 6/16. It always last 7-8 days though. 3 - I log them since the April. It has happened for a couple of years with a frequency of about 1 to 3 months 4 - I don't have any of those symptoms.

Keeping in view all the information you have provided, it seems that you have Idiopathic Chronic Fatigue. This is a very common condition found in as much as two-thirds of patients with chronic fatigue. The exact cause of this condition is not yet known. The symptoms usually include an inability to initiate activity (perception of generalized weakness, in the absence of objective findings), reduced capacity to maintain activity (easy fatiguability); and difficulty with concentration, memory, and emotional stability (mental fatigue).

Absence of good quality sleep can worsen the fatigue. So it is very important that you get good sleep.

You do not require any additional tests for now.

Management of this condition is mainly by cognitive behavioral therapy (CBT) and graded exercise therapy (GET). Iron supplementation, even in the absence of anemia has shown to help in reducing fatigue.





I have the following recommendations.

1. Please maintain good sleep hygiene practice and please do not over-sleep anytime.

2. You can start with iron supplementation with Ferrous gluconate 60 mg (Ferate [OTC] or Fergon® [OTC]), once daily with meals, for 1 month.

3. Please get in touch with your GP/ a therapist to discuss and start the above mentioned treatment options such as CBT and GET.
