Hard painful lump in butt after fall.

Hello,

Last week I fell down some wood stairs and hit my butt/lower back pretty bad. My left butt cheek became severely bruised, much worse the second day. Now I noticed two days ago, the bruising has gone away at one part, but now there is a huge (about the size of my hand), hard, painful lump. My butt cheek is even denting in a little where part of the bump is. Should I go to a doctor/immediate care center? I do not have insurance and am currently unemployed. I am asking if this is something that requires medical attention or just probably part from falling and will go away eventually. Trying to avoid paying a lot of money, but if it is something serious, I will.

I understand your concern.
From your description it seems like the injury part has a lot of inflammation and edema. It is secondary to injury. Butt area has a very big muscle called the gluteus maximus and any kind of injury will manifest with a lot of edema and swelling. With due time it will resolve.
It does not seem to be anything serious right now. I will advise you an anti-inflammatory medicine. You can take that for a couple of days and get back to me about the status of the swelling.
1) Tab. Ibuprofen 400 mg twice daily, after food for 3 days.
2) Any local anti-inflammatory spray can be applied locally to relieve the swelling.
Let me know if the swelling subsides on taking the medicine,

Hello,
I've taken the Ibuprofen for four days, like you said, but it's still the same. It may be a little smaller, but not by much. The bump is still super hard and painful in spots that I touch. The bruising is much better.

The swelling might take some time to resolve completely. However, if you have any of the following symptoms, it is better to see a doctor:
- Pain and restriction of hip movements
- Numbness/tingling anywhere on the same side lower limb
- Pain in the affected area even on rest
Presence of any of these symptoms indicate the lump to be a hematoma (collection of blood) which is compressive, within the gluteus maximus muscle. A simple hematoma resolves on its own in 2-3 weeks when the collected blood is reabsorbed.

If you do not have any of the above mentioned symptoms, then you can apply a warm/hot compress over the lump. Along with that, regular isometric exercise, that is tensing and relaxing of the muscles in that area will help in resolution. You can continue the Ibuprofen for another 3 days, if the pain is unbearable.
