I am 4 1/2 months pregnant, give or take. Baby measured 11 centimeter last week, grew from 5 centimeters since Januaty 3rd. On Friday, I fainted on the way to work, first felt nausea, and hot, sweating, trouble breathing, blurred vision and after blacked out for about 2 minutes. I have fainted in the past because I have IBS and when I go to the bathroom, which is once a week, about half the time, I faint. Now, everyday since Friday, I feel the beginning of the above symptoms. This past week the baby bump grew, a lot, and I have been having a lot of lower back pain, now in the past 2 days, I feel pressure in the vagina, almost like it is open. I am worried about 2 things. I saw doctors Friday after I fainted, my blood pressure changes a lot from standing, to sitting to laying down, and they did a sonogram and internal, so would that detect if I had a short cervix, or weak one? Is everything happening normal? I'm afraid I need a cerclage and the doctors here missed it! Is it recommended that I have a C-section since I have a history of fainting, and at times mini seizure like episodes after? I currently live in Italy, though I am American, and even though I speak the language, I don't speak it well enough for peace of mind.

Firstly, relax !
Having nausea, feeling hot and uncomfortable ( particularly since you are in Italy right now ) , mild breathlessness , even blacking out - these are quite common in pregnancy.

They happen due to the normal pregnancy associated changes in your body systems.
Could you upload the latest ultrasound findings, I can opine if the growth is alright or not.

Also let me know the date of your last menstrual period. From the fourth month onwards, the baby bump does start steadily showing and increasing.

Now, since you have had a history of fainting, it might just be the way you are.
Also, from the fourth month onwards, the uterus enlarges and puts pressure on the surrounding pelvic organs.

IF you are feeling pressure, cervical evaluation is a must. Yes, an internal digital examination and a transvaginal sonogram would definitely pick up a shortened , dilating cervix, ( providing they know how to look for it ! ).

A cerclage is in order if the cervix is shorter than 25 mm, or dilated significantly.

WHat you can do is schedule an appointment again with a reputed obstetrician, and specifically ask him / her to look for an incompetent cervix.

Also, with the mini seizure like episodes, a neurological examination should be done.IF it is normal, just fainting does not mandate a C section.
C section is more complicated in every sense, if you can have a vaginal delivery, you should aim for it.
You would be up and about faster.
Only the neurologist would be the best judge for that.

As a precaution, rest well, avoid stress, coitus and jerky travelling.
Discuss with your obstetrician regarding supplementation with progesterone, it quietens uterine contractions.

It uploaded before I wrote anything or uploaded the picture. I am not a smoker or drinker. I do not have diabetes or hypertension, though both my parents due. Last menstruation was October 25, but the Dr tells me that is impossible! It was lite, a different color and lasted maybe 2 days, if that. I had a vaginal exam by hand, without machines, would this show cervical problems? Is the sensation that I have a tampon in normal, or the numbness there? And where do I upload pictures to?

The feeling of a tampon there ALL the time, is not normal.
Yes, digital examination by hand also would show an incompetent or shortened cervix, but vaginal scanning is confirmatory.
You have uploaded the images, please upload the reports ( words ) that would show the weeks of pregnancy and the dates.
