Suffering from extreme fatigue with HIGH OXIDATIVE STRESS.

HI Doctor,

I have been suffering with extreme fatigue since July 2012, up till then I was very fit,
I played squash twice a week road cycle races, a normally busy person, then around
July 2012 I had to stop it all, I just couldn't cope, my wife was 4 months pregnant
and we thought it had sympathy pregnancy as I felt just as she did, flat!
then I started with other symptoms moody, anxious, forgetful, i get a rash on my thighs
and bum ( just bumps, like pimples), stressed
(I have never felt stressed in my life), using the wrong word in the middle of a sentence,
hard to concentrate, I can only focus on thing at a time, but not just while I am doing it, if
I have something on my mind I cant focus on any thing else till I refocus on something else.
I have always been a somewhat of an OCD person, but since I have been like this I get over the top
OCD,I catch any sickness around me, people at work tell me I am often look green when
I am not feeling well. I am tired, I need at least 10hrs of sleep to survive the day, and by the time I get home from work (08:00-16:00) I just want to veg on the couch, I have two kids one being two and the other is nine months which makes it hard to veg, I have very little libido,

I have seen my family gp many time, and he says he cant help me, I have seen a specialist and he ran many blood test and came back with nothing but "High oxidative stress" , mostly looking for adrenal fatigue, I have seen a blood analyst and my blood looks terrible under the microscope. I have gone through your question in detail and I can understand what you are going through.
From what you have stated it is quite clear that you are having a depressive episode. Depression can occur with regular stress and it can also occur biologically without any stress as well. However the symptoms that you have stated regarding fatigue, moodiness, reduced interest, anhedonia and others do suggest that its depression. You need proper treatment for the same. There are various effective therapies for depression like fluoxetine, venlafaxine, bupropion, mirtazapine or escitalopram and they are all very effective and mostly safe. You may also require some benzodiazepines like clonazepam or lorazepam for a short period like 2-3 weeks to reduce the anxiety and help with your sleep.
If you are averse to the drug therapies then there is also an option of cognitive behavioural therapy. This therapy is taken by either a psychologist or a psychiatrist in which the therapist identifies impaired cognitions (Thoughts) and try to correct them. They are 15-20 min weekly session and there should be around 8-10 sessions to help you with your illness.

Good Morning Dr. Srikanth Reddy Hope you are well? Thank you for your response, you are the first doctor that has told me that Depression could be the cause of all my problems, saying that the first drug that you listed “fluoxetine”, is the drug that my GP has given me to try last November 2014 but I stopped taking it in December, I have started taking it again for the past 3 weeks and see if it help. I Just have a few Questing?

  1. What type of medicine do you specialize in?
  2. Does my rash also point to Depression? Whenever I am at my worst or not feeling great my rash flairs up on my thighs and backside? If not what could be causing my rash, at the same time as my symptoms?
  3. Could I possible have cfs?
  4. I wake up n the morning with a metallic task in my mouth could this be me grinding my fillings? When I am feeling not so bad the task goes away?
  5. I have a mouth full of filling with both new type and older mercury based fillings some covering cavities and others root cannels could I not have an infection in one of my root cannels and this be causing my symptoms?   8. Ever since I have started taking fluoxetine again I get tension in my neck causing bad headaches is this normal?
  9. Ever since I can remember I have had piles, they seem to bleed from time to time could this not be poisoning my blood causing my symptoms?
  10. I am very concerned that I have something else wrong with me that no one has thought of and symptoms of Depression are just masking the real problem, until it get to the point something in my body fails!!
  11. All else failing would type of doctor would you recoment that i go see? i have seen 2 gp's and 1 specialist physician and a blood analyst, with no real joy!

Ans:Unlikely . The rash cannot be symptom of depression but feeling bad about rash can be exacerbated with depression.
  3. Could I possible have cfs?Ans:Its a possibility , but depression is the first diagnosis and should be treated first.
  4. I wake up n the morning with a metallic task in my mouth could this be me grinding my fillings? When I am feeling not so bad the task goes away? Ans: No. Unlikely to do so.
  5.   9. Ever since I can remember I have had piles, they seem to bleed from time to time could this not be poisoning my blood causing my symptoms? Ans: No blood poisoning will not lead to this gamut of symptoms.
  10. I am very concerned that I have something else wrong with me that no one has thought of and symptoms of Depression are just masking the real problem, until it get to the point something in my body fails!!Ans: If it is depression,then the symptoms should be coming down in 4 weeks time. Give it some time. Things will improve.
