Pain with burning sensation in hip travelling down to legs.

Resolved Question:

I'm a 29 y.o. female with right hip pain for the last 2 weeks. Pain sometimes is a burning sensation that radiates from the hip towards the front of the thigh. Sometimes the pain is felt in the back, around the sacroiliac joint. Relevant history is that pain started during my first pregnancy (have had 2 now) and got progressively worse. Pain started at sacroiliac joint and later at the pubic bone. Pain felt like grinding at the pubic bone and was severe (8/10) specially turning over in bed at night. By 5th month of pregnancy I had a waddling gait due to the pain. After internet research I self diagnosed myself with Symphysis Pubis Dysfunction and went to see a chiropractor. My OBGYN dismissed my pain as normal pregnancy aches. Chiropractor was familiar with the condition and greatly alleviated my pain and my gait returned to normal. No xrays were ever done due to pregnancy. After delivery my pain was gone, but returned as I got pregnant again 9 months after delivery. The symptoms were the same, so I resumed chiropractic care as soon as symptoms returned and had otherwise a very healthy and comfortable pregnancy. My youngest is now 13 months old and I had been pain free since delivery. Two weeks ago my menses restarted and that is when my symptoms came back and have been getting progressively worse. Today I did an xray and the finding that my pubic bone is misaligned is very upsetting and frightening to say the least.

I live outside the country, in the D.R. of Congo to be precise, and we don't have any specialists I could see in the immediate future. I requested the xray attached at a local clinic and if needed I could get additional xrays or a CT scan.

My questions at this point are: What exactly is happening at the pubic joint? What are possible treatments at this point? How soon should I seek treatment? How quickly will this continue to deteriorate? Is it possible I have other underlying problems (ex. hip problems, back problems, etc)?

From the history and X ray done I would give differential diagnosis of pelvic dysfunction which results from trauma our gait our posture during standing and day to day activity, especially if we sit with cross legged. associated symptoms are iliotibial band tightening. these conditions are purely amenable to physiotherapy and correction of mechanical factors. as a rheumatologist I would suggest few test to rule out inflammatory condition to be precise, please get HLA B 27 by PCR, ESR, CRP, Vit D 3 status.
X ray dorsolumbar and lumbosacral spine anteroposterior and lateral film and revert back to me.
